DETAILED SERVICES

Early Stage 

​

Home Safety Evaluation and Recommendations:  Our staff will perform a full extensive evaluation of your home environment and make recommendations based on the client's needs. (ie. Labeling cabinets, adding railings and safety bars, increasing light levels, stove guards and many more)
Providing Meaningful tasks: Our staff will evaluate client's past interests and activities and provide adaptive tasks/activities given the client's current cognitive status. (ie. Card games, crosswords, craft or woodworking projects, functional home tasks)
Reminiscence Work: Our staff will collect photos from the client's past to make life legacy books or videos for use today and after cognitive decline.  They can be used today for triggering of past memories and for assisting in grounding the client to memories that they can retrieve from their past. They can be used in the future for familiarizing outside caregivers to the client's past. Our staff will also utilize media (audio and visual) from familiar decades to facilitate discussions and increase mood.  These activities can help clients when they are experiencing anxiety or agitation brought on by their forgetfulness and confusion.
Home Exercise Programs: Our staff will provide a physical exercise program for maintaining client's strength, endurance and balance.  Clients and caregivers will receive handouts for use on their own.  Current research indicates that aerobic exercise can have neuroprotective effects on dementia.

 Safety Screenings and referrals for Driving:  Our staff will screen for physical limitations, visual impairments, decreased reaction time and cognitive impairments that effect driving and a client's safety behind the wheel.  If an "on the road" assessment is recommended, we will make recommendations or referral to a Driving Rehabilitation specialist.
Cognitive Training: Our staff will teach techniques to assist with memory and organization such as calendars, to do lists, written and visual instructions throughout the home and set memory timers for activities and medication dispensing.

Middle Stage

ADL (Activity of Daily Living) Skill Training:  Our staff will develop programs to assist clients to maintain the highest level of independence during ADLs.  We will provide necessary adaptive equipment or techniques to assure client's safety.  We will educate caregivers in the use of visual and verbal cues, demonstration techniques, partial physical assistance and problem solving in order to make tasks easier.
Adapt and Modify living situation:  Our staff will suggest alternative ways of completing tasks, assist in making the environment as safe as possible and work with clients/caregivers to modify home environment for safe and independent activities.  We will also assist clients and caregivers with situations of wandering and psychosocial behaviors in order to keep clients safe.
Cognitive Stimulation: Our staff will provide activities to continue to stimulate memory and problem solving with ADLs and household tasks.  We will evaluate and provide cognitive activities as the client's dementia symptoms progress.  We will engage the client in meaningful tasks that they can have success with.

Late Stage

Activities to decrease caregiver burden: Our staff will provide support and assistance to the caregiver through education and resources.  We will help with transfer training and passive range of motion exercises to assist clients and caregivers during basic ADL's such as Bathing, Dressing and Feeding.
Wheelchair and Seating Evaluations:  Our staff will assist caregivers in appropriate seating for the client's current physical stage.  From geriatric chairs to wheelchairs, we will find the most comfortable, engaging and safe seating situation for clients to be able to get out of bed.
Sensory Stimulation:  Our staff will employee a sensory program to enhance quality of life for our clients.  We will incorporate all the senses of auditory, visual, tactile, motion and smell and taste in our program.  The goal of this program is to engage cognitive presence and reaction from our clients.
